Surgically Induced Astigmatism (SIA) Calculator: Vector Analysis, Mean SIA and Toric IOL Rotation

Last updated: November 2026

Toric IOL calculators ask for your surgically induced astigmatism (SIA): the astigmatism your incision adds. It is not the change in the cylinder number; it must be worked out as a vector, because astigmatism has both size and axis. Enter keratometry before and after surgery for one eye, or paste a series to get your own average (centroid) SIA.

Before surgery

After surgery (at least 4–6 weeks)

How it works

  • Each astigmatism (size C at steep axis θ) is turned into a vector on a double-angle plot: x = C·cos 2θ, y = C·sin 2θ. Doubling the angle makes 0° and 180° the same point.
  • SIA = post-operative vector minus pre-operative vector, converted back to a size and axis.
  • For your personal SIA, use the centroid (vector mean) of at least 20–30 eyes, not the average of the magnitudes; the arithmetic mean overstates SIA because it ignores direction.
  • Typical temporal clear corneal SIA is about 0.2–0.5 D for 2.2–2.8 mm incisions; superior and scleral incisions behave differently.
  • Toric rotation: residual cylinder = 2 × C × sin(rotation). Each degree loses about 3.3% of the correction; at 30° the correction is lost entirely and the axis shifts.
