Something in the Eye

Foreign body first aid

Dust, sand, an eyelash, an insect or a speck of metal can get stuck on the surface of the eye or under the eyelid. Loose particles often wash out with tears or water. Anything that stays, or any particle that hit the eye at speed, needs an eye doctor.

First aid

Do

  • Wash your hands, then rinse the eye with clean running water or saline for several minutes, holding the lids open.
  • Look up, down and sideways while rinsing, or blink under water in a clean bowl.
  • Pull the upper lid out and down over the lower lid to sweep the inside of the lid.
  • If something is stuck on the coloured part of the eye, or you were hammering, grinding or drilling, cover the eye loosely and go to an eye doctor.

Don't

  • Don't rub the eye: it can scratch the cornea or push the particle deeper.
  • Don't use tweezers, a needle, a cloth corner or your fingernail on the eye.
  • Don't try to remove anything stuck in the eye itself.
  • Don't use numbing drops at home.

High-speed injuries are different

Hammering metal on metal, grinding, drilling, cutting tiles or an explosion can fire a tiny fragment into the eye. There may be little pain. This is an emergency: the fragment can cause infection or rust inside the eye. Cover the eye with a paper cup and go to an eye hospital the same day; the doctor may order a CT scan.

Rust ring

A metal particle on the cornea can leave a brown rust ring within hours. The doctor removes the particle and rust with a fine needle or burr under a microscope after numbing drops, and gives antibiotic drops or ointment.

See an eye doctor the same day if

  • You still feel something after rinsing
  • There is blurred vision, a cut or blood
  • The particle hit the eye at high speed (metal work, grinding, explosion)
  • The eye is very red, painful or sensitive to light the next day

Prevention

Wear safety glasses or a face shield for grinding, welding, drilling, gardening with power tools and chemical work. Ordinary spectacles are not enough.

Frequently asked questions

What if I cannot see the particle?

It may be under the upper lid or already washed out leaving a scratch. If discomfort continues after rinsing, see an eye doctor, who can turn the lid over to look.

Can I use eye drops to wash it out?

Clean water or sterile saline is best. Lubricating drops are fine afterwards for comfort.

Is it normal to feel something after it is removed?

A scratchy feeling for a day is common because the surface is scratched. It should improve steadily.

Why did the doctor order a scan?

If a fragment may have entered the eye at speed, a CT scan checks for a hidden intraocular foreign body. MRI is avoided until metal is ruled out.
