What is posterior capsule opacification?

During cataract surgery, your cloudy natural lens is removed and replaced with a clear artificial intraocular lens, which sits inside a thin membrane called the lens capsule. In some patients, this membrane gradually becomes cloudy over time — months or even years after otherwise successful surgery — causing blurry vision, glare, or haze that can feel just like the original cataract returning. This is called posterior capsule opacification (PCO), sometimes informally called a "secondary cataract," though it is not a new cataract and your original surgery hasn't failed.

How YAG laser capsulotomy works

A YAG laser is used to create a small, precise opening in the centre of the cloudy capsule, clearing the visual pathway so light can reach the retina normally again. The procedure is done entirely in-office — no incision, no stitches, and no operating theatre. Numbing eye drops are used, and most patients feel little to nothing during the brief procedure.

What to expect during the procedure

You'll sit at a laser system similar to the slit lamp used during routine eye exams, resting your chin and forehead against a support. You may see flashes of light or hear soft clicking sounds during treatment, but there's no cutting and no discomfort beyond the brief unfamiliarity of the sensation.

Recovery

Recovery is minimal. Many patients notice clearer vision within a day, sometimes sooner. A short course of anti-inflammatory eye drops may be prescribed, and a follow-up visit is recommended to check your eye pressure, since this is the main thing monitored after the procedure. Unlike the cataract capsule itself, once treated, this cloudy membrane does not grow back.
