Chalazion (Eyelid Lump)

A blocked oil gland in the eyelid

A chalazion is a firm, usually painless lump in the eyelid. It forms when one of the small oil (meibomian) glands in the lid gets blocked and the oil builds up. It is not an infection and not cancer. Many settle with simple home treatment within a few weeks.

Home treatment

  1. Warm compress: a clean cloth soaked in comfortably warm water (or a microwaved eye bag, tested on your wrist first) held on the closed lid for 5 to 10 minutes, 3 to 4 times a day.
  2. Massage: straight after, massage the lump gently with a clean finger towards the lashes for 30 seconds.
  3. Lid hygiene: clean the lash line daily with diluted baby shampoo or a lid wipe.
  4. Keep going for 3 to 4 weeks; most chalazia shrink.

Do

  • Wash your hands before touching the eyelid.
  • Stop eye makeup until it settles, and replace old makeup.

Don't

  • Don't squeeze or prick the lump.
  • Don't use steroid eye drops (for example dexamethasone, prednisolone or combination drops) unless an eye doctor has examined you: they can make an infection much worse.

If it does not settle

A chalazion that lasts more than a month, is large, presses on the eye and blurs vision, or keeps coming back can be removed with a small procedure under local anaesthetic (incision and curettage), done from the inside of the lid so there is no visible scar. Sometimes a steroid injection into the lump is used. Children may need a short general anaesthetic.

See a doctor promptly if

  • The whole eyelid becomes red, hot and swollen, or you get fever (possible cellulitis)
  • Vision is affected
  • In an adult over 50, a lump keeps coming back in the same place, bleeds or causes loss of lashes: it should be checked to rule out a rare eyelid tumour

Frequently asked questions

How long does a chalazion last?

Many settle in two to six weeks with warm compresses. Some last months and need a small procedure.

Do antibiotics help a chalazion?

Usually not, because it is not an infection. Antibiotics are used only if it becomes infected.

Will it come back?

It can, especially with blepharitis or rosacea. Daily lid hygiene and warm compresses help prevent new ones.

Is chalazion removal painful?

The lid is numbed with an injection first; you feel pressure but not pain. Most people return to work the next day.
